Have you seen your GP about your condition?

Are you male?

Have you been prescribed this before?

Is your hair loss symmetrical (same on both sides)?

Is your hair coming out in irregular patches or clumps?

Is your scalp inflamed, red, sore or itchy?

Are you currently being treated or tested for benign prostatic hyperplasia or prostate cancer or are you having your PSA monitored?

Are you aware that Propecia/Finasteride is only effective if it is taken continuously for at least 6 months?

Are you aware that in most cases Propecia/Finasteride produces it’s best effects after 2 years and is most effective in men when taken for at least 5 years?

Have you been diagnosed with any of the following? 

• Male Breast Cancer
• Hypogonadism (low testosterone level)
• Depression or suicidal thoughts

Are you taking any of the following medications? 

• Chemotherapy
• Antifungals

Do you agree to acknowledge the following:

• Propecia (Finasteride) is dangerous for women
• I will use a condom during sexual intercourse because Propecia (Finasteride) can harm an unborn baby
• I will not be trying to conceive within the next 12 months?
• I will ensure that my medication is kept away from pregnant women as they should not handle it

I am aware of the following:

• The MHRA has received reports of depression and, in rare cases, suicidal thoughts in men taking Finasteride / Propecia for male pattern hair loss.
• I will stop using Finasteride / Propecia immediately and inform a healthcare professional if I develop depression

• The treatment is for my use only
• I have answered the assessment questions truthfully and accurately
• I agree to the Terms and Conditions and I confirm that I am over 18 years old
• I am aware that I may be contacted to give further information via phone or email and if I am not contactable this will lead to a delay in receipt of my prescription
