Are you pregnant, breast feeding or intending to become pregnant?

Have you been prescribed Salbutamol before?

Has your asthma been reviewed in the last 12 months?

Are you confident you know how to use your inhaler?

Is your asthma usually well controlled?

Do you use your inhaler more than four times a day?

Have you been diagnosed with any of the following? 

• Overactive thyroid gland
• History of heart problems such as an irregular or fast heartbeat or angina
• High blood pressure
• Any serious medical condition which may require immediate hospitalisation

Are you taking any of the following medications? 

• Medicines for an irregular or fast heartbeat, such as propranolol
• Diuretics used to treat high blood pressure or a heart condition
• Medicines for ADHD

• I will contact my GP or MyMedsUK if I experience any side effects
• The treatment is for my use only
• I have answered the assessment questions truthfully and accurately
• I agree to the Terms and Conditions and I confirm that I am over 18 years old
• I am aware that I may be contacted to give further information via phone or email and if I am not contactable this will lead to a delay in receipt of my prescription
